The doctor was worried, JPKM program would reduce the number of their patients who in turn can affect the revenue decline. Some practitioners claim that was met by the program somewhat decreased the number of patients. Patients who had handled the practice doctors are now switching to the use of facilities or hospitals Purbalingga health centers.

Health development aims to improve community health status optimal. Environmental indicators, hygiene practices, and access to health services is an indicator both of the three indicators that have been formulated in the vision of healthy Indonesia 2010. Behavioral and environmental factors play an important role in improving the quality of health where disease incidence is the result of interaction between humans, the environment and cause disease (host, agent and environment). This factor, especially in developing countries the greatest influence on the emergence of health problems or health problems.

I examined the two groups of specialist doctors, namely for Diabetes and Kidney. My brother has a disease Hypertension, also was treated regularly since 7 years ago at a hospital in Bandung.

1. Introduction
From epidemiological studies in Indonesia found the prevalence of diabetes mellitus (DM) of 1,5-2,3% of the population over the age of 15 years, even in an epidemiological study in Manado found 6.1% prevalence of DM. Research conducted in Jakarta to prove the increase in prevalence. The prevalence of DM in urban areas in Jakarta rose from 1.7% in 1982 Thun to 5.7% in 1993. Similarly, the prevalence of DM in Ujung Pandang (urban areas), increase from 1.5% in 1981 in Thun to 2.9% in 1998.
